Prism for Stroke 1.1.1

PRISM is the first virtual care program in the United States dedicated to assisting stroke survivors. Developed by stroke specialists at Mount Sinai Hospital in New York City, a globally renowned stroke center, this program is now available to patients and caregivers nationwide.

The PRISM App enables users to easily communicate and exchange health information with their assigned PRISM care partner, a healthcare professional specially trained to support them throughout their recovery process.

Individuals who have enrolled in the PRISM program through their hospital or the PRISM website (myprism.ai) will receive login credentials to access the app. Upon installation and activation, the PRISM app empowers users to:

• Complete enrollment procedures
• Create a patient profile
• Capture and share blood pressure and heart rate data
• Follow daily to-do lists
• Access valuable stroke education resources
• Track essential stroke recovery details over time
• Schedule appointments with care partners
• Conduct video meetings with care partners
